Once you have been reviewed and accepted YOU MUST complete and send in the release form - located here -. ADDRESS YOUR CDS TO THE ADDRESS SUPPLIED ON THE RELEASE FORM AND FOLLOW THE INSTRUCTIONS TO THE LETTER OR YOUR CD WILL BE REFUSED.
